Athlete Forms and Information

Welcome to WHHS Athletics! All forms below MUST be completed in their entirety and submitted online in the student's Aktivate account. The account must be completed and cleared before any participation. Some common errors are listed below.

Common Errors on paperwork:

  • Missing dates – Especially the date of exam on the physical. Make sure your provider enters all of their information, including the date.

  • Missing athlete signatures in required locations.

  • Failure to provide explanations for “Yes” answers on pages 1 & 2 of the physical.

  • Failure to provide insurance information. If none, enter “No Policy.”

  • Forging medical documents is a Felony and will be reported. Providers are consulted when information is questionable and/or signatures do not match.
